Jazzhands Mobile Services LLC, operated by Jazmin Ramirez, provides mobile massage, nail care, and corporate wellness services in the Denver metro area. This policy explains how information is handled when you visit mobilejazzhands.com, send an inquiry, or follow our online booking links.
Information you provide
Our Corporate Wellness Request form asks for your company name, contact person, work email, preferred event date, and, optionally, estimated employee count and special notes or setup details. When you contact us by email or phone, we also receive the contact information and message details you choose to share. During service coordination, you may provide an appointment location and service preferences.
We use this information to respond to questions, prepare quotes, coordinate visits, provide requested services, and communicate about your inquiry or appointment. Please keep form notes limited to scheduling and setup needs. Do not include payment-card numbers, medical records, or other sensitive health details in the website request form.
Corporate wellness forms
The request form sends the information you enter to Web3Forms for processing and delivery to the business. Web3Forms and its service providers may process submission data and technical information for delivery, storage, security, and spam prevention. The form is an inquiry, not a confirmed appointment. Booking through Square
Our Schedule Now and appointment-booking links open Square Appointments in a new tab or window. Information you enter there is submitted to Square. Depending on the booking steps and services selected, this can include your name, email address, phone number, appointment details, and any additional information requested in the booking flow. If Square requests payment information, it is processed through Square.
Jazzhands receives the customer and appointment information needed to manage your booking and provide the service. Square also processes information under its own privacy notices, including technical data, cookies, communications, and transaction information where applicable.
